• When mixing drugs, use only a small amount of liquid or food
• Enteric-coated and formulated slow-release medications should not be crushed
• Remain with client until medication is swallowed
• Teach the client and family about any new medication
2. Identifying the patient:

• Use 2 methods for identification
• Do not use the patient's room #
• Check the patient's name and birthdate on their identification band
• Ask the patient to state his or her name and birthdate.
• Check the allergy bracelet
• Ask the patient about allergies
• Scan the patient's bar code on the identification band
3. Common drugs that may have a reactant:

-tylenol
-bronchodilator: for asthma, COPD, etc.
-coumadin:anticoagulant
-herbal preparations: may have adverse affects when mixed with other durgs
4. Assessment before admnistering meds:

• Learning ability of patient and family.
• Know patient's medical history.
• Know history of allergies.
