General: Answer 1) Note affect, appearance, and any notable distress (note if they arehappy, sad, emotion, dressed for the weather) 2) Assess orientation (alert to person, place, time, and situation) Ask for name, DOB,A&O x4, date and time, why they are here 3. Integument: Answer 1) Inspect for abnormal lesions and for overall integrity -skin pigmentation: consistent throughout, use back of hands: temperature is warm,texture is smooth, moisture is moist. Squeeze upper, lower arms, and ankles for pitting edema (no edema noted) (if they have edema, grade it) skin turgor is either good,fair, or poor,find clavicle:pinch skin, ifit flattensimmediately:good, 3 seconds:fair, more than 3 seconds: poor 4. Head and Neck: Answer 1) Inspect and palpate head and scalp for symmetry, shape,involuntary movements -look from front, side, and back. Head is symmetrical, normocephalic, and noinvoluntary movements -no pain/tenderness upon palpation, no masses and lesions 2) Inspect facialsymmetry of eyebrows, eyes, mouth, nasolabial folds(are symmetrical)
