Nursing Process (ADPIE) (also ANA standards)
● Assess- gather info about pt condition
● Diagnose- identify pt problems
● Plan- set goals of care and desired outcomes; identify nursing actions needed
● Implement- perform nursing actions from your planning
● Evaluate-see if goals and expected outcomes were achieved
Levels of Care:
● Primary- health promotion → office/clinic visits, nutrition counseling, family planning, etc
● Preventative Care- program based → cancer screening, immunizations, mental health
counseling
● Secondary- diagnosis & treatment of acute illness/injury → emergency care, acute med/surg
care, x-rays, CT scans
● Tertiary-specialized/ highly technical care → ICU care, oncology or burn centers
● Restorative- intermediate follow-up care for restoring health → home health, rehab, SNF
● Continuing- long-term or chronic health care needs → palliative or hospice care, adult daycare
Subjective Data- data from the client; what the client or family members say verbal description:
feelings, perceptions, and concerns. Collected through assessment.
Objective Data- observable and measurable data (physical exam, labs, diagnostic tests etc)
