summarize Mr. Granger’s current health state for the next nursing shift. Hint: Since Mr.
Granger is a new admission your summary should include a comprehensive, but brief,
review of all of your findings, not only abnormal results. To develop your note, type the

S – Mr. Granger was admitted directly from his primary care physician’s office for
therapy during his first exacerbation of stage Two right-sided heart problems. There was +2
pitted edema on the lower limbs, a new S3 pulse, buzzes on the airways, and dyspnea on exercise
and gasping. He exhibits a 10-pound weight increase in the previous three days, as well as
nocturnal sweating, SOB, and pulmonary edema.
